The risk of suicide attempts and death is highest within the first few weeks after a person is discharged from an Emergency Department or inpatient psychiatric unit, yet as many as 70 percent of suicide attempt patients never attend outpatient appointments. Therefore, continuity of care is critical for preventing suicide. This session will address the key components of a care transitions system. The session will also introduce the concept of the Zero Suicide Framework that health care systems can implement within their organizations. Presenter: Smita Varia

Panelists will share their personal stories of losing a loved one to suicide, followed by questions from the audience. Panelists: Robyn Toulotte, Amanda Ganoe and ?? Moderator: Nicole DeChirico
